Healthcare Directives

Honoring Choices Minnesota offers two free health directives which can be downloaded in either a PDF version (print it out and write in your information) or a Microsoft Word version (type into the document on your computer and then print it out).
  • Original Health Care Directive (8 pages long) - the comprehensive document to fully outline your choices and preferences regarding care to be given
  • *NEW* Short Form Health Care Directive (2 pages long) - a quick document to be completed if the original directive is not right for you at this time
Both have been carefully and collaboratively developed and reviewed by the Honoring Choices Minnesota Advisory Council, and are used at most health care centers in the Twin Cities area, and at an increasing number of health care centers throughout Minnesota.

We also offer health care directives in 4 translations: Hmong, Russian, Somali and Spanish.  NOTE: the translated directives use the 2012 Health Care Directive, found in English at the bottom of this list.  Please download that English version to be used side-by-side with the translated directive.
